This document provides an overview of root cause analysis techniques that can be used to identify the underlying causes of problems. It discusses objectives, an overview of root cause analysis, and specific techniques including why reality charting, Ishikawa diagramming, multivoting, and the importance-difficulty matrix. Participants are guided through exercises to practice applying these techniques to a case study. They are then provided with next steps and deliverables which include selecting an RCA process by May 13th and completing an analysis by July 15th.
