I’ve spent 13 years in various roles within the post-acute care space, and I’m encouraged by the evolution over the years. Between the shift to value-based care, a spike in demand for home-based care, and innovative technologies that help facilitate care delivery and outcomes, the next five years are promising. However, significant challenges remain. We still face fragmented care transitions, misaligned incentives, access barriers and capacity constraints – especially as our population ages and workforce shortages persist. Overcoming these challenges is essential to achieving better outcomes, improved patient experiences, and stronger provider engagement.
