  • ‘When we heard Alfredo’s story, we knew we had to help,’ recalls HammondCare team member Pratikshya. Alfredo had already spent more than 120 days in hospital and no aged care home felt able to take him. Pratikshya is manager of Quintral Cottage at HammondCare Cardiff, part of a national Specialist Dementia Care Program – which also became Alfredo’s welcoming new home. ‘The care he has been given there has certainly been special,’ said Alfredo’s son Paulo. • Palliative patient Caroline wanted to gather her husband and eight children for a very special family portrait session. Thanks to you – our donors – The Dreams Program made it possible. The Dreams Program is a HammondCare Foundation program that helps turn dreams into reality for end-of-life patients and their families when it matters most. With more than 4,000 palliative care patients each year, your generosity makes moments like Caroline’s possible. Together, we can bring comfort, joy and dignity when it matters most. • HammondCare is proud to have been selected to develop Canberra’s first dementia care village. Described by the ACT Government’s Suburban Land Agency as a ‘landmark project,’ the new Curtin Dementia Village on the site of the former North Curtin Primary School will feature 11 cottages and 48 supported seniors living apartments. Included in the plan is a range of onsite services and a wellness hub, for the local community to enjoy too. Our vision is to create a world-class integrated community that’s familiar and welcoming – where people living with dementia feel they truly belong, and where the local neighbourhood can feel part of something special. • Caroline’s grandpa, 98, legally blind and living with dementia, was being constantly moved between care homes and hospital. Then a referral to the Hospital to Aged Care Support Program, delivered by Dementia Support Australia and supported by HammondCare, changed everything – for him and the family. ‘It felt like an enormous weight had been lifted off our shoulders,’ shares Caroline. • Laurie, 91, told us she was ‘running out of strength’ caring for husband Nando who lives with Parkinson’s disease – before our Short Term Restorative Care team stepped in. Now she says, ‘I feel like myself again, and that means I can keep caring for him.’ Read Laurie’s and other stories about the people we’re privileged to serve in our 2025 Annual Report. • The Strength and Balance class at HammondCare Miranda is more than a fitness session—it’s an opportunity for connection. Mary, Vicki and Silia (pictured below with HammondCare team member Zeya) and their friend Soula attend the classes so they can continue to participate in their favourite activities, like gardening and hiking. During term break, Vicki organised a trip to nearby Kiama so the new friends could enjoy a day out together. Vicky and Mary also both live alone and Soula’s family were holidaying in Greece. ‘This blossoming friendship is testament to the power of our community allied health and therapy programs – promoting physical health, but also nurturing emotional wellbeing,’ shares Zeya. #CommunityCare #HealthyLiving #ActiveAgeing #WellbeingMatters #FriendshipAndFitness

  • Sally*, aged just 40, had only weeks to live and was too unwell to celebrate Christmas with the people she loved. HammondCare Foundation team stepped in to provide funding for a high tea celebration through The Dreams Project, an initiative that brings unique experiences to end-of-life patients and their families. Held in the garden kiosk at HammondCare’s Neringah Hospital, it was a team effort, with even the hospital maintenance team getting involved. A volunteer photographer recorded the special event for the more than 30 family members who attended. Sally died just a few days before Christmas, but not before her dream created precious lasting memories for her extended family. Ready to make someone’s dream come true? • HammondCare Caulfield resident and Vietnam Veteran Robert lives with PTSD and vascular dementia. As a result of his condition and lived experience, he often feels the urgent need to catch a flight – even attempting to climb out his window to get there in time. • ‘Sometimes it’s the smallest gestures that make the biggest impact,’ shares HammondCare Senior Clinical Psychologist Stefan. Stefan supports older Australians with complex mental health challenges – both at HammondCare Braeside Hospital and in the wider community. ‘I found a specialised group program for someone in my care, but the transport cost to attend was too much for them.’ ‘Thanks to HammondCare Foundation’s Care Fund, supported by generous donors like you, I was able to secure taxi vouchers to enable this person to attend the group each week.’ The program has had a profound positive impact on their mental health and overall quality of life.
