People experiencing homelessness face additional challenges such as financial limitations, lack of transportation to facilities, and stigma that make accessing the healthcare they desperately need extremely difficult. Street medicine programs, like Healthcare in Action (HIA) in California (a member organization of SCAN Group), provide a method to overcome those challenges and get people access to healthcare. Through their Community Connect partnership with Cedars-Sinai, HIA staff get access to patients’ comprehensive health records, allowing physicians to make informed decisions. Viewing and documenting patient information on-the-go through mobile devices helps improve continuity of care as patients see different teams. And with population-level data and analytic tools, HIA can identify patients who should be prioritized to drive better outcomes across an entire community. https://lnkd.in/dwsDbFuF

When entering the doctor’s office, patients should be focused on their health—not on paperwork and insurance information. Coverage Finder and Digital Insurance Card Exchange speed up registration, reduce errors, and minimize headaches so that patients can concentrate on what really matters. “From reducing time at the front desk by up to 50%, to ensuring their coverage is accurate and up-to-date, this makes insurance information one less thing patients need to worry about,” said Alan Hutchison, Vice President at Epic. Simplifying the patient experience and reducing administrative burden were at the heart of development efforts for these Payer Platform features, which Humana’s Chris Walker and Epic’s Ryan Bohochik presented at the Centers for Medicare & Medicaid Services (CMS) Connectathon last week. Clinicians in the hospital spend a lot of time searching through notes, results, and messages to get the full story of a patient’s stay. Integrated AI can help. To help care teams quickly orient themselves to the most important details of a patient’s admission, Inpatient Insights AI scans a patient’s chart and generates concise, timestamped summaries and discrete insights, such as why a medication was changed. After rolling out the feature at Riverside Health, users have reported significant decreases in time spent in various activities throughout the patient's chart, including a 22.5% decrease in time spent in the Notes activity, a 32.5% decrease in time spent in Secure Chat, and a 21.6% decrease in time spent in Results Review, adding up to meaningful time saved across teams.

Following Singapore’s migration to Epic, the National University of Singapore has conducted advanced one-day personalization courses to see if training would result in quantitative improvements. The researchers found that trained clinicians did indeed reduce the time spent across various functionalities, increased their use of SmartTools, and increased their use of Quick Filters.
