To assist CCPN practices experiencing Medicaid rate cuts, CCNC is immediately advancing 2024 earnings for all five prepaid health plans to provide additional revenue to member practices. Although final settlements from the health plans have not yet occured, it is our hope that this will provide some immediate financial assistance in the form of PHP Quality Incentive payments and shared savings. CCNC is distributing approximately $19 million in funds, which CCPN Advanced Practice Medical Home practices earned for outstanding 2024 performance. With prospective payments distributed earlier this year, over $24 million will have been paid out after this additional distribution. In NC, physician-driven, patient-centered care benefits more than 6,000 providers, 1.6 million patients and 9 million taxpayers.

The October issue of Business North Carolina spotlights Seven Peaks Family Medicine in West Jefferson, Ashe County, which was founded this spring by three young physicians who chose rural practice over big-city opportunities. Drs. Victoria Knox, Jason Karimy, and Jessica Stevens met during their residencies in Boone and realized they all shared a commitment to serving rural North Carolina. With support from Community Care of North Carolina, they secured a low-interest loan, built their 5,000-square-foot clinic, and launched a thriving independent practice. “The business of medicine is not something they teach you in medical school,” Knox said. “We couldn’t have done it without CCNC’s help.” Congratulations to the Seven Peaks team for advancing access to high-quality, community-based primary care in Western North Carolina. Check out CCNC’s new data brief: “Effectiveness of Care Management for Engaging Patients with Their Medical Homes” The evaluation explores how care management influences Medicaid patients’ engagement with their primary care providers. Key findings include: • Patients who received care management outreach were significantly more likely to visit their primary care provider than those who did not. • The longer it had been since a patient’s last visit, the greater the impact. For example, patients who hadn’t seen their provider in 4+ years were 68% more likely to reconnect due to care management.. Overall, care management shows the strongest results among patients historically less connected to their medical home – highlighting where focused efforts can make the biggest difference.
