Proud to share our latest episode of Saved by the Belle: Member Stories that Showcase Our Value, and Our Mission Episode 17: A Kidney Care Connection Mary is 68 years old with hypertension, diabetes, and stage 3 chronic kidney disease. On May 15, she was admitted into our Chronic Care Management program. During her visit with Belle, she shared with our clinician that she did not have a nephrologist. Our Care Manager immediately sent her a list of specialists to review. The next day, Mary selected a provider and asked Belle to help coordinate a referral through her PCP and schedule the appointment. By May 20, our team had obtained the referral and secured her first nephrology visit for July. We later confirmed that she attended and already had her follow-up scheduled. Because of timely coordination and follow-through, Mary is now connected to a nephrologist to help her better manage her chronic condition and prevent complications. This is just one of many ways Belle helps members get the right care at the right time. Stay tuned for the next episode of Saved by the Belle.

Proud to share our latest episode of Saved by the Belle: Member Stories that Showcase Our Value, and Our Mission Episode 16: A Pressure Check Meet Mr. Jones. He’s 78 years old, with a history of diabetes, hypertension, and coronary artery bypass grafting (CABG). During an in-home visit with our Belle Technician, his blood pressure was measured at 173/103. He reported no symptoms of elevated blood pressure but mentioned his doctors had stopped his antihypertensive medication six months earlier. By the end of the visit, his blood pressure remained elevated at 153/103. Though he was still asymptomatic, our Belle Care Manager notified his primary care provider about the readings and advised the member to go to the ER if any symptoms developed. The next day, our team followed up and confirmed that Mr. Jones had already seen his PCP, who restarted his blood pressure medication and instructed him to monitor his blood pressure at home. Belle will continue to monitor him in partnership with his PCP. Mr. Jones was at high cardiovascular risk. By catching these issues on his very first visit, our team helped him avoid potentially serious complications. This is just one of many stories of how in-home care, a dedicated care team, and close coordination with providers can make a life-saving difference.
