Have you signed up for the Hospital at Home Leadership Summit? Join Krista Drobac, Founder of Moving Health Home, in a discussion on SNF at Home. Hospitals across the country are facing a discharge crisis, with patients ready for discharge sitting in inpatient beds for days and sometimes weeks and other patients needing care boarding in the ER. This panel will examine “SNF at Home” as one solution to this problem. Quality post-acute care is crucial for patient recovery. With the number of patients requiring post-acute care in an environment with reduced options, and the resulting bottlenecks in hospitals due to difficulty of finding available beds for discharge, the “SNF at Home” model presents a valuable opportunity to expand post-acute care capacity. This integrated approach allows services to be delivered directly to patients in their own homes, promoting recovery in a familiar environment. https://lnkd.in/gP8qYbsE
How do we solve healthcare’s discharge crisis? Across the country, patients ready to leave the hospital are waiting days or even weeks for a post-acute care bed — while new patients board in the ER. One emerging answer: SNF at Home. At the Hospital @ Home Leadership Summit (Dec 3–5, Orlando), Krista Drobac, Partner at Moving Health Home, will lead a discussion on the policy, operational, and technology challenges shaping this next frontier of home-based care. This panel will explore how “SNF at Home” can: » Expand post-acute capacity without adding physical beds » Improve patient recovery and caregiver wellbeing » Navigate the legislative and regulatory frameworks needed to scale Hospitals can’t wait for new facilities — they need new models. Title: “Why Patient Experience Is the New Benchmark in Healthcare” In today’s world, healthcare is no longer just about treatment — it’s about the experience. Patients expect empathy, clarity, and comfort at every touchpoint — from reception to recovery. Clinics that invest in communication, transparency, and follow-ups see higher trust, retention, and satisfaction rates. • A friendly tone at reception • Clear explanations from doctors • Timely updates and smooth processes These small steps create a big difference in how patients feel about their care. Because in the end, patients may forget what you said, but they’ll never forget how you made them feel. 💙 #Healthcare #PatientExperience #MedicalCare #ClinicManagement #HealthAwareness #PatientCare

What Concierge Medicine Means to Us at VTR Health After more than a decade in primary care, I’ve learned that the best medicine happens when doctors have time to truly listen. That’s what concierge medicine allows — time to think, time to talk, and time to care. It’s not a luxury; it’s common sense. At VTR Health, we focus on building real relationships through our telemedicine practice. Our direct‑pay, subscription model gives patients easier access, longer appointments, and ongoing follow‑ups — the way primary care used to be. Concierge care means patients feel heard, supported, and understood. It’s medicine centered on people, not paperwork — restoring trust, connection, and continuity. This is the future of healthcare, built on something timeless: genuine relationships between doctors and patients. #VTRHealth #ConciergeMedicine #PrimaryCare #Telemedicine #PatientCare #PreventiveHealth

Accessibility is one of the most underestimated levers in healthcare strategy. Most people think it’s about convenience. Although that is true, it’s about outcomes, trust, and equity. Patients who can access care quickly and locally are more likely to stay on track with treatment, avoid complications, and build lasting relationships with their providers. Three things I’ve seen hold true across the industry: • Proximity matters: Care closer to home improves follow-through. • Speed matters: Urgent access prevents small issues from becoming major ones. • Simplicity matters: The easier it is to book, check in, and follow up, the more likely patients are to engage. Healthcare leaders should ask: Are we designing care around our systems, or around our patients? The difference shows up in both patient outcomes and long-term loyalty. What strategies have worked for your team to bring care closer to the people who need it most? #HealthcareLeadership #Accessibility #ValueBasedCare #PatientFirst #PhysicianLedCare
