The post-acute care landscape is evolving rapidly, and the shift toward value-based models is quickly becoming a critical part of the formula for helping improve patient outcomes while supporting both payers and providers. In my recent conversation with Maria Castellucci on Modern Healthcare’s “Healthcare Insider” podcast, I discussed the current challenges of the post-acute care system and how OneHome is uniquely positioned to contribute to the solutions. Our goal is always to ensure high-quality, coordinated care for patients, especially as demand grows and workforce challenges persist.
