The time for value-based care (VBC) might finally be at hand, according to the chief physician executive of the American Hospital Association. Speaking at last month’s HLTH USA conference in Las Vegas, Chris DeRienzo, MD, MPP, said momentum for VBC is growing. “I see it all over the place in hospitals I visit, large and small around the country,” he told Chief Healthcare Executive. “This need to serve our communities is a really deep and passionate driver of everything that we do in hospitals and health systems. So, I honestly could not be more optimistic about what we see through the windshield here in the years ahead.” DeRienzo acknowledged that the long-promised shift to VBC has been slow and halting and still faces challenges, including a shortage of primary care physicians. The systemic reasons affecting GP access ‘Smaller practices have been shown to increase patient outcomes and satisfaction. It is likely that this is due to greater knowledge of individual patients, continuity being key, but even low-level continuity with previous knowledge of patients can be extremely helpful in managing patient care. This is much more likely in smaller practices, where patients are more likely to have greater continuity of care. It’s also more likely that smaller practices have linked knowledge of patients across the whole team, with conversations between admin, reception and clinical staff being more likely to occur.’ https://lnkd.in/epW5jY5E

The real cost of ignoring patients after discharge is higher than most hospitals realize. Studies show that nearly 20 percent of hospital readmissions are preventable, many of which occur because patients receive insufficient follow-up care. Poor post-discharge engagement contributes to complications, medication errors, and avoidable emergency visits. Hospitals lose thousands per patient episode due to penalties and lost reimbursements under value-based care models. This is why early post-discharge engagement is paramount. Hospitals that implement structured follow-up programs, including automated check-ins, remote monitoring, and targeted outreach, have reduced 30-day readmissions by 15 to 25 percent. Patients who receive early engagement are more likely to adhere to treatment plans, attend follow-up appointments, and recognize warning signs before complications arise. Operationally, proactive post-discharge programs free up staff and streamline workflows. Case managers can focus on high-risk patients while routine follow-ups are automated. The result is better continuity of care, higher patient satisfaction, and improved clinical outcomes. More importantly, fewer readmissions reduce penalty exposure, increase performance under value-based reimbursement models, and protect revenue streams. High-risk patient populations, such as those with chronic conditions or complex social determinants, benefit most, ensuring care is both effective and cost-efficient. Early post-discharge engagement is quickly becoming a critical strategy that aligns patient safety with revenue protection, turning follow-up care into a key driver of hospital profitability.
