What if hospital care came to you — instead of you going to it? That’s the idea behind Hospital at Home programs now expanding in places like Canada and Australia. Patients are formally “admitted,” but everything happens at home — IV meds, oxygen, cardiac monitoring, virtual physician rounds, and daily nurse check-ins. In one South Australian study of over 2,000 home-based admissions, mortality was 0.2%, and 28-day readmissions were 4.9% lower than traditional hospital care. In Canada, 81% of participants reported a positive experience. It’s important to note — this isn’t Home Health. Home Health supports recovery after discharge. Hospital at Home replaces the hospital stay entirely. It reminds me of when I first helped launch Mobile Integrated Healthcare. Back then, we were redefining where care begins. Hospital at Home takes that same philosophy further — meeting patients where they live, when they need it most. Technology, teamwork, and trust are making it possible — and I believe it could be one of the biggest shifts in how we deliver acute care in the next decade.

“I used to see the same GP every time.” Before Covid, many patients regularly saw the same clinician. Today, that reality is different. List sizes have grown and many practices have merged. As a result there are more roles involved in primary care, and many of them can safely manage new, acute symptoms. So if seeing the same GP every time isn’t always realistic, how do we drive continuity of care? Continuity matters most for long-term conditions, ongoing symptoms and anything where subtle changes really count. Having one clinician in these situations builds trust and helps to prevent duplication of testing or explanation. But for new, more straightforward problems, another clinician may be the most appropriate person to see. Paramedics and advanced nurse practitioners can help patients be seen sooner, protecting continuity for those who need it most, without reducing quality of care. Continuity doesn’t have to mean the same face. It’s about using people’s skills wisely and balancing clinical capacity across the team.
