- [Narrator] On January 28th, 1986, teachers around the world turned on their televisions so that their students could watch the launch of the Space Shuttle Challenger. This was a special day because a teacher named Christa McAuliffe was part of the crew. However, disaster struck just 73 seconds after blast-off. The rocket exploded and no one on board survived. A presidential select commission was assembled to investigate the cause of the tragedy, which was traced to the failure of a relatively small part, a faulty O-ring. As part of the investigation, a social psychologist named Irving Janis, who specialized in group decision-making, testified before Congress. While most people were eager to point the blame at a single irresponsible individual, Janis insisted that no one involved acted out of malice, ignorance, or even incompetency.
