Our ThinkWell Kenya team recently visited Kenya's Principal Secretary of Medical Services, Dr. Ouma Oluga to discuss Cervical Cancer elimination efforts in Kenya. ThinkWell is supporting the MOH to develop the cervical cancer elimination action plan. We discussed the government's plans to switch from a 2 dose to a 1 dose vaccine, as per WHO and KENITAG guidance. ThinkWell looks forward to this switch, which is anticipated to save US$21.4 Million over the first five years, which could potentially fund 2.75 million supplemental HPV vaccinations. This can further increase vaccine coverage from the current 18%, against Kenya’s target of 90%. The HPV vaccine is highly cost effective against preventing cervical cancer, the leading cause of cancer deaths among women in Kenya. Joining us were colleagues from National Cancer Control Program - Kenya and Gates Foundation. A week after #IHEA2025, we’re highlighting Obaid Khan's presentation on translating UHC policy into district-level reality, shared during one of ThinkWell’s sessions at the congress. In his presentation, “Implementing UHC Benefit Package in Pakistan: From National Framework to Local Realities,” Obaid shared insights from Sindh’s Tando Allah Yar district, where only one-third of top-priority UHC interventions were fully available at baseline—revealing critical service delivery gaps. 📌 The EPHS pilot shows that UHC can’t wait for perfect conditions. Aligning services with real budgets creates a practical path for turning policy into impact on the ground. 📌 District localization is essential, budget alignment bridges the policy-practice gap, and political ownership is key to sustained implementation. Read how ThinkWell supported the provincial government in translating a DCP3-based Essential Package of Health Services (EPHS) into budget-aligned implementation plans using a direct costing approach. 🌍 International Health Economics Association IHEA Congress 2025 Reflections: From Fragmented Financing to Local Realities Just back from an inspiring week at IHEA Congress 2025 in Bali, where I had the privilege to present Pakistan’s journey in strengthening primary health care (#PHC) financing across two critical sessions. 🎯 Session 1 (July 20): Fragmented PHC Financing in LMICs "In a moment of major change across our field, ensuring coherence and strengthening country PHC financing systems has never been more critical." I shared Pakistan’s case study from Sindh and Khyber Pakhtunkhwa, highlighting how PHC fragmentation - while sometimes offering flexibility - creates long-term barriers to equitable and efficient care. 💡 Key insights: 📌 Retain strong government purchasing and oversight - never outsource stewardship, only operations. 📌 Direct facility financing enables bottom-up planning but needs capacity building and financial autonomy. 📌 Fragmentation is nearly universal but deeply contextual; complex & dual systems complicate coordination and accountability. 📌 Way forward: align budgets with responsibilities, improve spending visibility, integrate funding streams, and embed robust performance parameters in contracts to make purchasing truly strategic. 🎯 Session 2 (July 23): Implementing UHC-Benefit Package in Pakistan "From National Framework to Local Realities" From the EPHS pilot in Tando Allah Yar, I shared how ThinkWell’s locally grounded, fiscal-realistic costing approach is helping Sindh’s DoH turn a DCP3-based EPHS into budget-aligned, district-level plans while also strengthening the public sector’s capacity for financial management - a key outcome for sustaining UHC reforms. 💡 Key learnings: 📌 The EPHS pilot shows UHC can’t wait for perfect conditions. We’re proud to be part of this timely workshop on supply chain financing in LMICs, hosted by the Center for Global Development with ARC - Africa's Public Health Supply Chain Institution. Understanding country-specific contexts is essential for meaningful reform, and this session highlighted how complex supply chain financing requires tailored solutions, especially in the face of shrinking donor funding, fragmented systems, and decentralized governance. Anooj Pattnaik, Murira Felix, and Dr Shabana Haider took part in the session on centralisation vs decentralisation of procurement functions. The discussion explored which functions work best at each level and the conditions required for success, drawing from cross-country research and experiences in the counties of Kenya and the provinces of Pakistan. The team’s reflections were informed by our work on essential medicines and health supplies financing. At #IHEA2025 yesterday, we hosted an in-depth workshop on the financing of supply chains in LMICs. A critical and topical issue, as donors rapidly cut their support to supply chains. We were privileged to host 35 people from more than 15 countries, to share their expertise, critique a novel draft conceptual framework, consider how better governance can overcome the fragmentation of supply chains, and debate the pros and cons of decentralising financing. This session was part of a joint Working Group co-chaired by Center for Global Development and ARC - Africa's Public Health Supply Chain Institution.
